Rechecked SVRSevern Valley Railway news, yes it was. Jubilee bank holiday, Summer '77. 80079 was the passing loco, Driver/Director's name not mentioned, probably just as well!

Derailed front and rear axle. Centre ones stayed on despite being slightly askew to the line of the track. I recall the hat being passed round for donations from the spectators when re-railed. Although I do know who the driver was; he's still about so to save further possible embarrassment I agree best not mentioned or my faux pas of a similar era may get aired.
