===Northwood Crossing collision===
On 25 November 2000 [[LMS Stanier 8F 48773]] was involved in collision with an Astra van on [[Level crossing at Hill Farm, Northwood Lane | Northwood Crossing]], fortunately with no fatalities involved. HM Railway Inspectorate later confirmed that the Railway was not at fault, independent witnesses having testified that the driver had passed the flashing red lights when the train was only a few yards from the crossing.<ref>SVR News 134</ref>
===Fishermen's Crossing collision===
On 16 August 2004 [[Fishermen's Crossing]] was the scene of a collision between [[LMS Ivatt Class 2 46443|Ivatt Class 2 46443]] and a van which drove onto the crossing without stopping, the crossing gates having been left open by a previous user.<ref>SVR News 148</ref> The van was carried some distance down the track, with the driver suffering injury<ref>[http://orr.gov.uk/__data/assets/pdf_file/0017/1664/railsafety04.pdf HSE's annual report on railway safety 2004 – ORR] Retrieved 8 June 2015</ref> though fortunately not fatal. H.M. On 21 April 1991 [[42968|Stanier Mogul 42968]] was running tender first on a passenger train when the leading left wheel of the tender derailed by flange climbing. The derailment occurred on a right-hand curve over the site of an embankment slip at [[Folly Point]] which was subject to a temporary 5 mph speed restriction. The SVR's investigation found that the weight distribution between the wheels of the tender was not set up correctly following maintenance. The RAIB investigation into the 2009 derailment at Hampton Loade referred to this incident under "Previous occurrences of a similar character"<ref name=RAIB>[https://www.gov.uk/raib-reports/derailment-on-severn-valley-railway Rail Accident Investigation Branch] (retrieved 6 January 2017)</ref>.
===Hampton Loade derailment===
On 28 September 2009, a derailment occurred when the tender of [[LMS Ivatt Class 4 43106]] hauling a down passenger train became derailed as it approached [[Hampton Loade]]. There were no injuries. The immediate cause of the accident was the leading right-hand wheel of the tender flange climbing over the rail at the site of a track twist. An incorrect spring had been fitted to this axle of the tender.<ref name=RAIB/> [[BR 4345 Tourist Standard Open]] was also damaged.
